Job Description

About the role

We are looking for Community Registered Nurse Hospice to join our Hospice @ Home team supporting patients and whānau in their own home. This position is a great fit for a dedicated and capable professional with expertise in palliative care, who has a strong interest in delivering homebased care. Responsibilities will include assessment, care planning, treatment and outcomes for people with specialist palliative care needs.

 

As a Community Registered Nurse, you will deliver high-quality, person-centred care in the home. Your role will include:

  • Providing direct care and support to patients and their caregivers throughout the palliative care journey
  • Assessing and managing symptoms, including pain and medication support
  • Supporting a range of care needs, while providing respite and emotional support to whānau

We have two positions to fill,

  • a permanent position with 32 hours per week
  • a fixed term position with 20 hours per week until November 2026.

The shifts are rostered and rotated scheduled from 1:30 pm to 10:00 pm, Monday through Sunday.

 

Desired Skills and Experience

About you

You will bring:

  • Registered Nurse with a current NZ annual practicing certificate
  • Ideally, at least two years’ post-registration experience, with a background in palliative or community nursing (training provided for the right candidate)
  • Liaise with the Multidisciplinary Team and other Health Care Partners
  • Have a right to work in New Zealand
  • Hold a full NZ driver's license
