Job Description
About the role
Mary Potter Hospice is seeking a compassionate and experienced Allied Health Team Leader for our Community team. You will provide clinical and operational leadership to our Community Allied Health team. This pivotal role supports high-quality, equitable, patient-centred care through strong coordination, workforce leadership, and multidisciplinary collaboration across community psychosocial and therapeutic services.
Working closely with the Community Services Manager and Nursing Team Leader, you will:
Inspire and lead the day-to-day coordination of our Community Allied Health team, fostering a culture of collaboration, excellence, and compassionate care
Champion effective multidisciplinary team (MDT) meetings, supporting seamless patient flow, admissions, and discharges to ensure people receive the right care at the right time
Ensure timely, holistic psychosocial and therapeutic assessments and care planning that place patients and whānau at the centre
Grow and empower others through coaching, mentoring, and meaningful professional development
Drive continuous quality improvement, contribute to innovative service development, and help shape future workforce planning
Actively promote equitable access to services and honour Te Tiriti o Waitangi in everyday practice
Desired Skills and Experience
About you
You will bring:
Proven experience and a strong understanding of healthcare or a related field
Confident leadership skills, with the ability to coordinate and facilitate effectively
Sound knowledge and practical expertise in psychosocial and/or palliative care practice
Excellent communication skills, with the ability to build meaningful relationships
Experience working across a variety of clinical and/or community-based settings
A genuine understanding of Te Tiriti o Waitangi and its application in practice
A positive, team-focused approach with a collaborative mindset
